Both improvements and developments should result in increased rent reimbursement for the GP practices who own the site. Given rental income is a major factor in determining the capital/sale values for GP surgery sites, increases in rent in turn can increase the value of the property.
Extensions and most major improvements ALWAYS require the prior approval of the NHS before any works commence. If you do not secure prior approval, it is likely that the practice will not receive NHS rent reimbursement (notional rent/current market rent) on the new accommodation/improvements.
